Please take this form to make changes to your child’s booking pattern in the Tiny Tots Nursery: the form is available 24/7, and can be used to book Swap Sessions that your child has previously missed, or when you know your child will not be attending in the future.

Remember that your child has an annual swap allocation of up to two weeks pro-rata (eg, if they attend 3 days per week, they are entitled to 6 swap days per year).  See terms and conditions. Your last invoice states the number of swaps available at the top.

This form cannot currently be used for Extra sessions.
